Can anyone recommend somewhere I can get this stitched into the back seats, to match the front?

It must be exact so obviously a professional with lots of experience -who would you recommend?

And the approx. cost?

Try Stu's Trim in Manukau, as far as cost, well you'll have to ask them :)

Stu's Trim Shop does do a good job, I've seen quite a bit of work from them and it is always good.
